Your therapist should easily be able to give you a provisional diagnosis upon your first visit. 

  • Request a Superbill: Speak with your therapist to confirm that they will be able to provide a superbill. Most licensed counselors should be able to provide this. You may request a superbill after each session, or have multiple sessions combined on one document. Ensure it includes all the necessary information like your name, therapist's details, session dates, and diagnosis codes.

  • Complete Your Claim Form: Your insurance provider will have a claim form that you need to fill out. Attach the superbill to this form, ensuring all the details match. 

  • Submit the Claim: Send your completed claim form along with the superbill to your insurance company. You can typically submit this online or through the mail, following the instructions provided by your insurer.

  • Follow Up: After submission, keep track of your claim's progress. Contact your insurance company if you haven't received reimbursement within a reasonable timeframe.
